An exciting opportunity has arisen for a Care Management Nurse with a forward thinking organization in London. Your role will involve proactively identifying patients who have gaps in care and are most at risk for inappropriate use of the healthcare system. As the Care Management Nurse you will be required to close those gaps through education, support and active nurse monitoring.
You will have direct communication and collaboration with the GPs, Specialists, Consultant Doctors, Community Team and Patient in order to facilitate a plan of care with a goal to prevent admission and re-admissions.
The ideal candidate will need good organizational skills, effective time management, ability to manage multiple priorities, and strong teamwork skills. Its important you have a good knowledge of acute trust s discharge planners, care management and disease management. This is a challenging opportunity for highly skilled Registered Nurses to influence healthcare outcomes.
